2006 Volunteer Income Tax Assistance (VITA)

The Episilon Psi chapter of the Beta Alpha Psi fraternity will be helping International Students with their Income Tax returns.  All VITA assistance will take place in BuC 103 on the University of Utah Campus. All assistance will be given on a walk-in basis, no appointment is necessary.

**Assistance with non-resident returns (1040NR) will begin on February 16th

Before you come in:

1. Download and print the Intake and Interview Sheet available at:

2.  Fill out page one of the Intake and Interview Sheet.

3.    In addition to the Intake and Interview Sheet, make sure you bring the following:

    *        Proof of Identity


    *        Copies of all W2, 1098, and 1099 forms


    *        Social Security (SSN) Card or Individual Tax Identification Number (ITIN) for all individuals to be listed on the return.


    *        Child care providers' identification number


    *        Taxpayers' banking information (voided check and/or savings deposit slip) for refund deposits


    *        Estimated tax payments made, etc


    *        Amounts of other income
